A Clinical Evaluation on the 28 Cases of Intubation Granuloma




Abstract
Intubation granuloma is a rare complication of endotracheal. It is a condition, however, which is becoming more frequent owing to the increasing role of intubation in modern surgery. Despite of its therapeutic value, several post-traumatic
complications
such as laryngeal edema, ulceration and granuloma are developed following intubation. Understanding of the pathogenesis of intubation granuloma is a value not only in the prophylaxis but also in treatment of this disease. For 9 years, 1985

to
1994 December, the authors had experienced 28 cases of intubation granuloma in our department and following results were obtained.
1) Incidence of intubation granuloma during 9 years was about 0.043%.
2) Age distribution showed mostly in 20 to 50 year old group (75%). Sex incidence of male to female is 3:11, predominantly female.
3) The sides of lesion were bilateral in 11 cases and unilateral in 17 cases (left side 10 cases, right side 7 cases).
4) Duration of intubation was 1-3 hrs in 17 cases, 4-6 hrs in 4 cases, more than 3 days in 4 cases.
5) Suspension laryngeal microsurgery was performed in all patient, but 1 patient was re-operated due to persistent granuloma.
Following above results, we experienced 11 cases of bilateral granuloma (35%) and 10 cases of left side granuloma (40%) among the unilateral 17 cases granuloma. And we suggest that intubation granuloma predominantly occur in premenopausal woman,
and
intubation granuloma do not relate to the duration of intubation at sexually active woman. Therefore we suppose that vocal cord mucosa contained some steroid hormonal receptor which responsible for development of granuloma. (Korean J Otolaryngol
